Yes, this form must be properly notarized per Board of
Directors resolution. |

I just did my recertification, do I still need to do
annual reporting? |

Yes, annual reporting is a separate requirement that must
be completely done by all members despite the possible
overlap with recertification. |

What should I disclose? |

Everything! If you have any question at all, disclose.
Send a short explanation and we will follow up with a letter
asking for follow up information based on the disclosure. |

I already disclosed something. Do I have to disclose it
again? |

Yes. Obviously you won't have to go through the rigorous
Standards Committee review if it is something we have
already approved; but, you do have to disclose it. The best
way to do this is to attach your copy of the approval letter
and refer to the prior matter by name. |

Why do I have to repeatedly disclose a matter that's
already been approved? |

Our forms don't allow for prior disclosures to be
omitted. Following the simple procedure above, you can
easily remind us of prior disclosures. |

Can I amend or alter the language in your forms and check the boxes? |

No. Our forms cannot be amended or altered in any way.
Please don't check an applicable box and send a letter
stating why you can't check a box or boxes. |

Oops. I forgot to attach an explanation as to why I
couldn't check a box or boxes. What's going to happen? |

Your annual report will be sent back and you will be
asked to attach an explanation. Your annual report will not
be processed without it! |

Oops. I just sent in my disclosure, checked all the
boxes and remembered I did get a reprimand. What should I do? |

The Standards Committee and Board of Directors takes
non-disclosure very seriously. Please disclose everything.
Read the forms carefully. Remember, a certification can be
revoked on the basis of non-disclosure! |

What happens if I don't send in my annual report by the
deadline? |

Annual Reporting is required by the Board of Directors.
Non compliance with annual reporting will cause revocation
of your certification. |

I am on emeritus status. Do I still need to do annual
reporting? |

Yes. It is important we have this from every member
whether they are emeritus or not. In addition to our own
rigorous standards, we are accredited by other agencies. We guarantee to many of those agencies that we have an annual report from all members. |
